To do so, you must make an appointment using the form available at www.sefcarm.es.<\/p>\n<p align=\"justify\">Online, using the &quot;Generic Application Form&quot; (procedure code 3458) available on the electronic headquarters of the Administration of the Autonomous Community of the Region of Murcia, which can be accessed through the website https:\/\/sede.carm.es. In this case, Annex I and, where applicable, Annex II must be attached to the generic application form. These must be electronically signed by the applicant, after completion, signatures by family members, and printing in PDF format.
